Accepted Insurance: Moda, Aetna, Providence, United Healthcare, and Regence Blue Cross Blue Shield of Oregon.

Individual Therapy: $225 per session

Out-of-Pocket Sliding Scale: $150 - $185 per session

Out-of-network billing: For insurance I cannot accept directly, your plan may have an out-of-network outpatient mental health benefit which means they may reimburse you for a portion of the cost. If you do have out-of-network coverage, a “superbill” (the bill they require) can be provided for reimbursement. For this option, please call your insurance and verify your plan has outpatient mental healthcare benefits for out-of-network providers before scheduling a consultation.     • Is there a separate deductible for out-of-network services? If so, how much is it and how much have I met so far?

    • Is pre-authorization or a referral required to use my out-of-network benefits for outpatient mental health?

    • How do I submit claims for reimbursement if I pay out of pocket?

    • Are there any limits on the number of sessions per year or restrictions on provider credentials (for example, LMFT, LPC, PhD psychologist)?

    • What is the usual turnaround time for reimbursement once I submit a claim?

    Just to confirm, can you tell me what the CPT codes would be for outpatient therapy reimbursement under my plan? I believe individual therapy is usually 90834 or 90837, and couples therapy is 90847—is that right?

This estimate outlines the expected costs of services over a specific period of time and is based on the information available at the time it is provided. Your actual charges may differ if your needs change.

You may request a Good Faith Estimate at any time, or one will be provided before services begin.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill. Additional information about your rights is available at www.cms.gov/nosurprises or by calling 1-800-985-3059.
